With Gleam, ABN 91 219 621 079, operated by Julius Lee, a registered oral health therapist, provides professional teeth whitening services from G08/420 Canterbury Road, Campsie NSW 2194.

Because we provide a health service and hold health information, we are bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs), and by the Health Records and Information Privacy Act 2002 (NSW) and its Health Privacy Principles - regardless of our size. This policy explains how we handle your personal and health information.

1. The information we collect

  • Personal information: your name, date of birth, contact details (phone, email, address), and payment details.

  • Health (sensitive) information: your medical and dental history, information relevant to your suitability for whitening, your treatment and consent records, and any clinical notes or photographs taken with your consent.

Health information is "sensitive information" under the law. We collect it only with your consent and only where it is reasonably necessary to provide your treatment safely.

2. How we collect it

We collect information directly from you wherever practical - through our online booking and intake forms, our returning-client update forms, in conversation, and at your appointment. If we ever need to collect information about you from someone else (for example, another health practitioner), we will seek your consent first, unless the law allows otherwise.

We use the following service providers to collect and process information on our behalf: Acuity Scheduling / Squarespace (bookings and new-client intake), Jotform (returning-client update and consent forms), and Square (payments).

3. Why we collect it

To assess your suitability for whitening; to provide treatment safely; to keep accurate clinical records; to manage your appointments, payments and communications; and to meet our professional, insurance and legal obligations.

4. How we use and disclose it

We use your information only for the purposes above, or for a directly related purpose you would reasonably expect. We do not sell your information, and we do not use it for unrelated marketing without your consent.

We may disclose your information: to another treating practitioner with your consent; to the service providers listed above, who are required to protect it; where disclosure is required or authorised by law; or where there is a serious threat to someone's life, health or safety.

5. Overseas storage

Some of our service providers (including Acuity / Squarespace and Square) may store or process information on servers located outside Australia, including in the United States. Where this happens, we take reasonable steps to ensure your information is handled consistently with the APPs. Our returning-client forms (Jotform) are configured to store data in the Australian region.

6. Storage and security

We take reasonable steps to protect your information from misuse, loss, and unauthorised access or disclosure. Signed consent forms are held securely at ProHealth Medical; clinical records and related documents are kept on secured, access-controlled devices; and information held online is protected by our service providers' security measures.

7. Accessing and correcting your information

You may ask to access the personal and health information we hold about you, and ask us to correct it if it is inaccurate, out of date, or incomplete. Contact us at withgleamco@gmail.com, and we will respond within a reasonable period. In limited circumstances we may need to decline access (for example, where the law requires it), and we will explain why in writing.

8. How long we keep it

We retain health records in line with our legal and professional obligations โ€” generally for at least 7 years from your last visit, or, if you were under 18 at your last visit, until you turn 25.

If you have a concern about how we have handled your information, please contact us first at withgleamco@gmail.com so we can try to resolve it. If you are not satisfied with our response, you can contact the Office of the Australian Information Commissioner (oaic.gov.au) or, in NSW, the Information and Privacy Commission NSW (ipc.nsw.gov.au).
